Job Description

We are seeking a compassionate and dedicated healthcare professional to join our palliative care team. The role focuses on supporting patients with life-limiting illnesses by managing symptoms, alleviating pain, and enhancing quality of life. Beyond medical care, the position emphasizes emotional support for both patients and their families, ensuring dignity and comfort throughout the care journey.

Key Responsibilities
Clinical

Deliver palliative care services in a community setting, ensuring comprehensive and patient-centered support.

Liaise with referring doctors to ensure continuity of care and appropriate treatment.

Participate in planning, implementing, and promoting ASSISS Palliative Services (APS) and ADOPTs programs.

Collaborate with the Head of APS on patient care needs and address arising challenges.

Provide ongoing monitoring of patients through regular visits and assessments.

Offer bereavement support and follow-up to families after a patient’s passing.

Professional & Procedural

Conduct financial assessments to prioritize patients requiring free healthcare services.

Perform comprehensive evaluations of medical history, physical condition, and psychosocial needs to develop individualized care plans.

Communicate treatment options, care goals, and advance care planning with patients and families.

Provide psychological and emotional support to patients and their loved ones.

Guide patients in making informed end-of-life decisions and ensure preferences are respected.

Collaborate with a multidisciplinary team including oncologists, nurses, social workers, and chaplains to deliver holistic care.

Apply pharmacological and non-pharmacological pain management strategies to maximize comfort.

Assist in preparing advance directives and living wills for patients.

Stay abreast of palliative care advancements and contribute to research, education, and training within the healthcare community.

Qualifications

Palliative Care Practitioner (Nursing):

Diploma in Nursing.

Registered with the Malaysian Nursing Board with a valid Annual Practicing Certificate (APC).

Palliative Care Medical Officer:

MBBS/MD or equivalent medical degree from a recognized institution.

Registered with the Malaysian Medical Council (MMC) with a valid APC.

Requirements

Possess own transport for community-based visits.

Strong clinical decision-making and problem-solving abilities.

Excellent interpersonal skills with a people-oriented approach.
