State Registered Nurse (OT, Medical & Surgical Wards)

We are seeking a dedicated and compassionate Registered Nurse to deliver high-quality patient care while ensuring clinical excellence and patient safety. This role involves comprehensive patient assessment, care planning, implementation, and continuous evaluation in accordance with established nursing standards and protocols.

Key Responsibilities
1. Patient Assessment & Monitoring
Conduct holistic patient assessments, including physiological, psychological, emotional, sociocultural, and spiritual needs
Gather relevant patient information from multiple sources such as patients, families, and medical records
Monitor and document changes in patient health status, particularly those affecting Activities of Daily Living (ADLs)
Maintain accurate and timely patient records to support continuity of care
2. Care Planning
Develop individualized nursing care plans in collaboration with patients, families, and multidisciplinary teams
Set measurable goals and expected outcomes based on patient conditions
Establish evaluation criteria to ensure effectiveness of care plans
Ensure all care plans align with clinical protocols and best practices
3. Implementation of Nursing Care
Deliver patient-centered care based on priority needs and established protocols
Assist in diagnostic and therapeutic procedures, including pre- and post-procedure care
Administer medications safely and accurately in accordance with medical prescriptions
Promote and maintain patients’ Activities of Daily Living (ADLs), including:
Respiratory care (e.g., oxygen therapy, positioning)
Nutritional support (e.g., diet management, IV therapy)
Rest, sleep, and mobility balance
Infection control and patient safety practices
Psychological well-being and emotional support
4. Patient Safety & Quality Care
Ensure a safe and therapeutic environment through adherence to infection control and safety protocols
Uphold patient dignity, confidentiality, and ethical nursing practices
Provide compassionate care that supports patients’ emotional and psychological needs
Collaborate effectively with healthcare teams to optimize patient outcomes
5. Additional Duties
Perform other responsibilities as assigned by management from time to time
Job Requirements
Diploma or Degree in Nursing from a recognized institution
Minimum 3 years of clinical nursing experience (preferred)
Possession of a valid Annual Practicing Certificate (APC) and registration (LJM)
Post-basic qualification will be an added advantage
Willingness to work on rotational shifts
Based in or willing to work in Kota Damansara
