Clinical Pharmacist

As a Clinical Pharmacist at Fe Del Mundo Medical Center, you will play a crucial role in optimizing medication therapy and ensuring the safe and effective use of pharmaceuticals for our patients. Responsibilities:

• Review and assess medication orders for accuracy, appropriateness, and potential interactions.
• Collaborate with healthcare providers to develop and implement patient-specific medication therapy plans.
• Provide drug information and education to healthcare professionals, patients, and their families.
• Monitor and manage drug therapy to maximize therapeutic outcomes while minimizing adverse effects.
• Participate in multidisciplinary rounds and collaborate with physicians and other healthcare team members to optimize patient care.
• Conduct medication reconciliation to ensure continuity of care during transitions between healthcare settings.
• Supervise and train pharmacy staff on best practices and the latest developments in pharmaceutical care.
• Stay informed about new drugs, dosage forms, and therapies to ensure up-to-date and evidence-based practices.
• Ensure compliance with regulatory requirements and maintain accurate and complete patient records.
• Contribute to quality improvement initiatives and participate in pharmacy-related committees.

Qualifications:

• Bachelor’s or Doctor of Pharmacy degree from an accredited institution.
• Licensed Pharmacist in the Philippines.
• Clinical pharmacy experience in a hospital setting is preferred.
• Strong knowledge of pharmacotherapy, pharmacokinetics, and drug interactions.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.
• Detail-oriented and committed to maintaining high standards of patient safety.
• Demonstrated commitment to continuous learning and professional development.
