Job Description
Job Responsibilities:
Interview, evaluate, and classify patients.
Provide social case management and interventions for patients facing social, emotional, or environmental challenges affecting their medical care.
Assist patients who are abandoned, neglected, or victims of abuse.
Support patient adjustment to the hospital environment during treatment.
Offer counseling and moral support to patients, their relatives, and hospital employees.
Contact relatives of patients who are abandoned in the hospital.
Explain hospital policies to patients regarding admission procedures.
Implement continuous quality improvement strategies using the Plan-Do-Check-Act model.
Attend Social Worker Development Seminars and other job-related conferences.
Participate in regular staff development meetings.
Prepare monthly reports for the Vice President for Hospital Services, Chief Medical Officer, and Director for Quality and Patient Services.
Assist patients in need of financial aid from accredited partner institutions.
If assigned to the PCSO-ASAP Desk:
Explain PCSO requirements to patients or their representatives.
Ensure the validity and completeness of documents.
Input data into PCSO software and submit scanned documents.
Inform patients about approved financial assistance amounts.
Prepare and forward necessary documents for claiming Guarantee Letters from PCSO.
Coordinate with the Credit and Collection Department and submit monthly reports to PCSO.
Perform follow-ups with PCSO when needed.
Carry out additional tasks assigned by the Senior Social Worker.
Job Requirements:
Bachelor’s/College Degree in Social Work.
Must hold a Professional License (Board Passer).
At least 1 year of experience in a related field.
Preferably 1-4 years of experience in social or counseling services.
One full-time position available.
