Job Responsibilities:<\/p>\n
Interview, evaluate, and classify patients.
\nProvide social case management and interventions for patients facing social, emotional, or environmental challenges affecting their medical care.
\nAssist patients who are abandoned, neglected, or victims of abuse.
\nSupport patient adjustment to the hospital environment during treatment.
\nOffer counseling and moral support to patients, their relatives, and hospital employees.
\nContact relatives of patients who are abandoned in the hospital.
\nExplain hospital policies to patients regarding admission procedures.
\nImplement continuous quality improvement strategies using the Plan-Do-Check-Act model.
\nAttend Social Worker Development Seminars and other job-related conferences.
\nParticipate in regular staff development meetings.
\nPrepare monthly reports for the Vice President for Hospital Services, Chief Medical Officer, and Director for Quality and Patient Services.
\nAssist patients in need of financial aid from accredited partner institutions.
\nIf assigned to the PCSO-ASAP Desk:
\nExplain PCSO requirements to patients or their representatives.
\nEnsure the validity and completeness of documents.
\nInput data into PCSO software and submit scanned documents.
\nInform patients about approved financial assistance amounts.
\nPrepare and forward necessary documents for claiming Guarantee Letters from PCSO.
\nCoordinate with the Credit and Collection Department and submit monthly reports to PCSO.
\nPerform follow-ups with PCSO when needed.
\nCarry out additional tasks assigned by the Senior Social Worker.
\nJob Requirements:<\/p>\n
Bachelor\u2019s\/College Degree in Social Work.
\nMust hold a Professional License (Board Passer).
\nAt least 1 year of experience in a related field.
\nPreferably 1-4 years of experience in social or counseling services.
\nOne full-time position available.
