Clinical Pharmacist

Emilio Aguinaldo College Medical Center is hiring a Clinical Pharmacist!

Requirements:
– Must be a Registered Pharmacist in the Philippines with at least 6 months of experience in Clinical Pharmacy
– Dispenses medications to patients according to their prescription orders and ensures they receive the right medication and dosage
– Collaborates with physicians, nurses, and other healthcare professionals
– Monitors and evaluates the patient’s response to the medication therapy, including any side effects or adverse reactions
– Provides patient education and counseling on medication therapy
– Maintains accurate records of medications dispensed and ensures compliance with regulatory requirements
– Compounds and prepares medications in accordance with prescriptions and regulations
– Participates in drug inventory management, including ordering and restocking medications
– Provides drug information and consultation services to healthcare professionals and patients
– Conducts medication therapy reviews and performs drug utilization reviews to identify opportunities for optimization of drug therapy
– Provides support in medication-related research and quality improvement initiatives
– Performs other related duties as assigned
