Responsibilities and Accountability of a Junior Consultant:

  1. The Junior Consultant on duty is expected to be knowledgeable about all patients under his/her care and shall prioritize critically ill and unstable patients.
  2. All referrals to the Junior Consultant on duty should warrant a bedside evaluation and assessment with corresponding documentation in the chart.
  3. A comprehensive history and physical examination must be completed within forty-eight (48) hours from admission, covering present illness, past medical history, family, social and personal history, and OB-GYNE History for patients.
  4. The duty Junior Consultant should inform the attending physician of the patient’s condition and subsequent management.
  5. Given an inevitable event, telephone orders must be countersigned before the end of duty, otherwise, should be avoided.
  6. The duty Junior Consultant in the ward must ensure that the Attending Physician is informed of admissions, review laboratory results, ensure appropriate ongoing management, and update/document the patient’s status/well-being in progress notes.
  7. When attending to a critical patient in the Emergency room and ward, the Junior Consultant is responsible for initiating critical care management – Advanced cardiac life support (ACLS), which may include performing intubation, cardiopulmonary resuscitation, and close monitoring.
  8. During ambulance conduction, the consultant on duty in the ward is responsible for proper coordination with the other institution, ensuring the safety and well-being of the patient during transport, and continuous monitoring of the patient while undergoing diagnostics and treatment.
  9. For Pediatricians: Responsible for assessing and monitoring the newborn from catch to rooming in and discharge.
  10. For Internal Medicine: Responsible for assessing and monitoring patients undergoing tests in the Cardiopulmonary unit, provided with incentive per patient monitored.
  11. Bedside endorsement of the patients in the ER, NICU/ICU, Cardio-Pulmonary Unit, Hemodialysis Unit, and the ward is required from the outgoing consultant on duty to the incoming consultant. No post is allowed to be empty without an incoming duty consultant.
  12. The consultant on duty is held accountable for medical history taken, physical examination, diagnosis, and management given to a patient without any referral to the attending physician. Any incident during the tour of duty requires an incident report submitted to the Chief of Clinics/Medical Director within twenty-four hours (24 hrs.).
  13. All ER OPD consultations should be referred for follow-up and referral to appropriate OPD consultants within 48 to 72 hours upon initial ER consult.
