• Conduct comprehensive assessments of patients’ health conditions.

• Monitor and evaluate patient progress and adjust care plans as needed.
• Provide patient education on health management and preventive care.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Registered Nurse or Bachelor of Science in Nursing Graduate.
• Good communication skills with the ability to interact effectively with patients and multidisciplinary teams.
• Strong sense of responsibility and discipline.
• Experience in conducting health assessments is a plus.
